Preparing childcare enrollment documents before your child’s start date can prevent delays and make the first day calmer. California-licensed childcare programs must collect specific information so they can care for children safely and contact the right people when needed.
Common California childcare enrollment documents
- Identification and emergency information: parent or guardian contacts, authorized pickup persons and emergency contacts.
- Child’s preadmission health history: medical conditions, allergies, developmental information and routines that affect care.
- Physician’s report: when required for the child’s age and program.
- Immunization record: documentation that meets California requirements or an applicable lawful exemption.
- Consent for emergency medical treatment: authorization to obtain emergency care when a parent cannot immediately be reached.
- Parents’ rights and required notices: signed acknowledgments supplied by the licensed provider.
- Enrollment agreement and program policies: schedule, tuition, attendance, health, pickup and other operating terms.
Information to gather before you begin
Have your pediatrician’s contact information, two reliable emergency contacts, names of every authorized pickup person, allergy details, medication instructions, preferred hospital and your child’s current immunization history. Write names and phone numbers consistently across all forms.

- Do not leave required signature or date fields blank.
- Use current phone numbers and update them whenever they change.
- Tell the provider about allergies or special health needs before the first day.
- Confirm which forms require a physician’s signature.
